The O-ring supplied with the valve is too thick. It won't allow the valve to properly and completely seat to the neck of the cylinder. When the fill pressure approaches, or exceeds 3,000PSI, the pressure will blow out the o-ring and dump the pressure rapidly in a very dangerous, and uncontrolled manner. You will need a #116 o-ring for the valve to seat properly against the neck of the cylinder. The thickness of a #116 measures 2.62mm. The o-ring that came with my valve was over 3mm thick. Too thick!!
